This module identifies the best practice methodology for developing reentry and transition case plans. Participants will learn the core elements of a comprehensive plan to determine reasonable conditions of probation, treatment and program options to meet the developmental needs of youth to increase the likelihood of successful reentry into the community. This module explores effective practices that involve the youth and family in developing the plan for reentry. Participants will gain the skill of communicating expectations for the youth’s behavior aligned with the collaboratively developed plan for reentry.
