Pair Team connects vulnerable patients to local care teams that help them overcome barriers and get convenient access to the right physical, mental, and social services. Pair Team handles the entire patient/member journey from initial outreach to the coordination of clinical and social services. We have a robust virtual care team, a network of community-based organizations, and a technology platform that is purpose-built for high level care delivery and coordination, including those transitioning back into the community. *We are 100% funded through CalAIM and the Enhanced Care Management (ECM) program.
